0630 – Instructor arrival @ Coopertown Middle School
0630 – 0715 – Classroom Set Up – A/V & Materials, Etc.
0700 – Walk- up registrations
0730 – Opening Ceremonies
0800 – Classes Begin
1130 – Lunch Begins
1230 pm – Classes Resume
1630 – Class wrap-up including hands – on
and Equipment clean up, trash etc.
1700 pm – Raffle & Door Prizes, & closing comments

TERMS AND CONDITIONS:
I the above named student planning to attend live fire or technical rescue training or any physical activities has no known medical or physical conditions that would prevent participation in any or all of the physical activities which may be required by the course that he or she may be taking.
Also the above named student if attending a course which requires PPE or SCBA will be required to bring his or her own which shall meet NFPA 1971 standard on Protective Ensemble for Structural Fire Fighting and NFPA 1981 standard on SCBA at the time of purchase by the student or the sponsoring Agency.
Also facial hair must comply with NFPA 1500 5-3.9 to participate in any class requiring the use of an SCBA.
I, the above named student, intend to be legally binding for myself, my heirs, executors, administrators, and assigns, Herby waive any and release all rights and claims for damages and injuries which I may suffer as the result of attending classes and / or training exercises.
The above named student, acknowledge that I have read and completed the above application and understand that I will be ask to sign a Waiver of Liability the day of the school In consideration for participation.
